New wellness program launched

Guided2health has introduced a new program called Guided2Health Wellness, which aims to reduce chronic health conditions by targeting the behaviors that cause them.

The program works with an employer's overall benefit program and is made up of five components:

  • Online: Features proprietary and personalized health risk assessments, cost management tools from Subimo, monthly and quarterly employee newsletters and custom reporting.
  • Coaching for Change: Features personalized nurse phone coaching for employees who enroll in the program and an individualized behavioral change action plan and educational materials for each employee participant.
  • At Work: Includes onsite health fairs and biometric screenings with findings for aggregate and individual reporting.
  • Results: Offers fully integrated reporting of health risk assessments, ranking against benchmarks and population improvements over time. Also included is a customized executive summary incorporating recommendations by a nursing analytics team and clinical account coordinator.
  • Support: Features support from a clinical account coordinator, targeted employer/employee communications tools and participation tracking for incentive programs.
