Up to 99% of behavioral health listings in some health plan directories are 'ghosts'

At one plan, just 8.8% of the providers listed were inactive.
At a majority of the plans reviewed, at least 33% of the providers listed were inactive.
Investigators have suggested that the government could help by creating a national provider directory.
