A California lawmaker is seeking to rein in addiction treatment centers and dialysis providers accused of profiteering off vulnerable patients by collecting millions of dollars in inflated medical claims.
Evidence is mounting that Medicaid's rapid expansion under the Affordable Care Act has far outstripped the government's ability to monitor the taxpayer money it turns over to insurers.

In early October, an executive at one of the nations largest physician-practice management firms handed her bosses the equivalent of a live grenade a 20-page report that blew up the company and shook the world of managed care for poor patients across California.
Sutter Health intentionally destroyed 192 boxes of documents that employers and labor unions were seeking in a lawsuit that accuses the giant Northern California health system of abusing its market power and charging inflated prices, according to a state judge.
The state Department of Managed Health Care criticized Anthem for systemic violations and a long history of flouting the law in regard to consumer complaints.
As soon as news surfaced last week about the potential merger of CVS Health and Aetna, all eyes turned to the looming threat from Amazon.
